Referral Information
Who is eligible for the program?
FolkTime members are asked to meet the following criteria:
* An 'Axis One' diagnosis (major depression, bipolar disorder, schizophrenia)
* 18 years old or older
* Be under the care or supervision of a mental health provider or social worker.
To provide a safe and supportive community, we accept members who have:
* A desire to participate in a community focused on recovery
* The ability to be with others in a positive way that promotes mutual benefit

How Can I attend?
If you are interested in attending FolkTime, we will need a referral form to be filled out by a caseworker or health provider. It is available below as a PDF. If needed, a FolkTime Staff member can also email, fax, or mail a copy to you.
